Tadalafil vs sildenafil: which fits your life

Both are well-studied treatments for erectile dysfunction. The real difference is timing: one works for hours, the other for up to a day and a half.

Same family, different timing

Sildenafil (the active ingredient in Viagra) and tadalafil (the active ingredient in Cialis) both belong to a family of medications called PDE5 inhibitors. They help blood flow to the penis when you're aroused. They don't create desire on their own, and they don't cause an erection without arousal.

Where they differ is how long they last, and that shapes how each fits into your life.

Sildenafil: plan an hour ahead

According to its FDA label, sildenafil is usually taken about an hour before sex, and it can be taken anywhere from 30 minutes to 4 hours before. A high-fat meal slows it down: on average it takes about an hour longer to reach its peak.

People who like it tend to want something they take only when they need it, with a shorter window.

Tadalafil: a longer window, or a daily dose

Tadalafil's label shows it improved erectile function for up to 36 hours after a dose, and it can be taken with or without food. That longer window means less planning around the moment.

Tadalafil is also approved at a low daily dose. Taken at about the same time every day, it's meant to keep you ready without timing anything.

Side effects

The most common side effects of both are headache, flushing, indigestion and a stuffy nose. The labels also list some differences: sildenafil can cause temporary changes in vision, most often a color tinge, and tadalafil more often causes back pain and muscle aches. They're usually mild and pass.

Safety first

These are the rules that matter most, whichever one you take:

  • Never take either with nitrates (such as nitroglycerin) or with riociguat. The combination can drop blood pressure dangerously.
  • Get medical help right away for an erection lasting more than 4 hours.
  • Sex puts some strain on the heart. Your clinician checks your heart health and medications, including alpha-blockers, before prescribing.
